You will first meet with a patient navigator who will take your eligibility information, discuss your needs and provide referrals. The appointment typically takes one hour.  Your information is kept completely confidential.  

For your Welcome Appointment, please bring the following documents:

•  Proof of where you live

•  Proof of your income

• Proof of insurance (if available)

• Proof of your HIV diagnosis

•  A photo ID

•  A list of your HIV medicines

Please arrive 15 minutes early for your appointment to fill out the required forms or print the New Client Forms below and bring the completed forms to your appointment.

If you are applying for services and have no income or are unable to provide documentation showing how you manage, please use this form as documentation.
